PREPARING FOR INITIAL VISIT

Our office wants to make your visit is as easy and hassle free as possible. Preparing in advance for your first visit will ensure a smooth registration and a more pleasant experience.

We ask that you call ahead and provide us with your full name, address, phone number, date of birth, telephone number and insurance name and ID number.  We will do our best to check your eligibility for services, but it is your responsibility to verify our participation with your insurance plan prior to your visit.  If you need a referral, please make sure you have it with you when you come in for your appointment.  Any co-payment must be paid prior to seeing the doctor.

You must present your insurance card upon your arrival to the office.  Minors under 18 years of age must be accompanied by a parent or legal guardian.  If you are feeling sick the day of your visit, we ask that you call us to reschedule your appointment.

WHAT TO EXPECT

You will need to complete the registration forms listed below upon your arrival.  To save time you may print these forms ahead of time, complete them at home and submit them when you arrive at the office.

You can also come in 20 minutes prior to your appointment and complete the forms in the office.  Make sure to bring a list of all your medications, allergies, medical and surgical history, pharmacy information, and primary care physician’s phone number and address.

A photo, for identification purposes by staff and doctors, will be taken for your chart.  You will then be taken to a treatment room and asked to take your shoes and socks off.  The doctor will ask you to describe your symptoms, perform a thorough foot exam, may order x-rays that are done in the office, as well as any other labs or studies.

Your physician will interpret these findings and will develop a treatment plan which can include non-surgical options such as orthotics, bracing, physical therapy, injections, change in shoe gear, etc.…  Outpatient surgery and referrals may also be discussed.

NEW PATIENT FORMS

The following forms will need to be reviewed and submitted on your initial visit.  You may print and complete these forms prior to your visit.  This will accelerate the registration process and decrease wait times.
